Use the pillow as the illustration shows. Notice she has some of the pillow past her neck. If you make the mistake of sleeping on that thin edge instead of moving your head to where hers is, you won't get the neck comfort you need.
My only quibble: I paid full price. If this product isn't on sale, wait till it is! The amount of money that people in pain have to spend on helpful devices is already way over budget...so wait for a sale.
A wonderful help, and it also prevents some of the effects of snoring...according to my DH, an unwilling audience. Now, when he hears me, he rolls me back onto the comma pillow. Problem solved!
